    Basic Dosage Calculations and Conversions

    Overview

Basic dosage calculation involves determining the correct amount of medication to administer based on a provider's order. This process requires comparing what is ordered (desired) to what is available (have) and accounting for the quantity of the supply. Accuracy in these calculations is fundamental to patient safety and the prevention of medication errors.

    Key nursing concepts

    • Identification of the desired dose prescribed by the provider.
    • Verification of the dose on hand available from the pharmacy.
    • Conversion of units to ensure the numerator and denominator match.
    • Calculation of the volume or number of tablets to be administered.
    • Application of rounding rules specific to the medication type.
    • Re-verification of the final answer against the original order.

    Important nursing considerations

    • Always verify the drug label against the Medication Administration Record (MAR).
    • Ensure the unit of measurement requested matches the unit on the supply.
    • Perform the calculation independently before checking with a colleague if required.
    • Use a zero before a decimal point (e.g., 0.5 mg) to prevent errors.
    • Never use a trailing zero (e.g., 5.0 mg) as it may be misread as 50.
    • Double-check all calculations for high-alert medications like insulin or heparin.

    Patient teaching

    • Explain the name and purpose of the medication being administered.
    • Inform the patient of the dose they are receiving in easy-to-understand units.
    • Encourage the patient to ask questions if the pill looks different than usual.
    • Instruct the patient on the importance of adhering to the prescribed schedule.
