    Bowel Elimination

    Overview

Bowel elimination involves the physical and chemical processes of waste excretion from the gastrointestinal tract. Nurses must assess bowel sounds, frequency, and stool characteristics to determine gastrointestinal health. Interventions often focus on maintaining regularity, managing incontinence, and providing ostomy care.

    Key nursing concepts

    • Normal bowel sounds occur every 5 to 15 seconds.
    • Constipation is defined by infrequent or difficult passage of hard stool.
    • Diarrhea leads to metabolic acidosis and fluid volume deficit.
    • Fecal occult blood testing (Guaiac) detects hidden blood in stool.
    • Enemas are used to stimulate peristalsis or deliver medication.
    • Ostomy care requires maintaining skin integrity around the stoma.

    Important nursing considerations

    • Verify medical history for contraindications such as bowel obstruction.
    • Auscultate for bowel sounds in all four quadrants before and after.
    • Monitor for electrolyte imbalances especially with frequent liquid stools.
    • Position patient on the left side (Sims position) for enema administration.
    • Assess for skin breakdown in the perianal area.
    • Stop the procedure if the patient experiences severe cramping or bradycardia.

    Patient teaching

    • Increase dietary fiber intake to 25-35 grams per day.
    • Maintain adequate fluid intake of at least 2 liters daily unless contraindicated.
    • Do not ignore the urge to defecate to prevent chronic constipation.
    • Engage in regular physical activity to promote intestinal motility.
