Pediatrics
Pediatric Musculoskeletal Nursing
Overview
Musculoskeletal disorders in pediatrics include congenital defects, acquired injuries, and chronic neuromuscular diseases. The focus of nursing care is to promote mobility, prevent complications of immobility, and support normal growth and development. Nurses must also be experts in neurovascular assessment to identify complications like compartment syndrome early.
Key nursing concepts
- Children's bones are more porous and heal faster than adult bones.
- Developmental dysplasia of the hip requires early detection for successful bracing.
- Scoliosis screening is vital during the adolescent growth spurt.
- Cerebral palsy is a non-progressive permanent disorder of movement and posture.
- Muscular dystrophy is a progressive weakening of the skeletal muscles.
- Compartment syndrome is a surgical emergency following trauma or casting.
Important nursing considerations
- Monitor for the 'Gower sign' (using hands to walk up legs) in Duchenne muscular dystrophy.
- Maintain strict immobilization and check neurovascular status for children in casts.
- Assess for the five P's of compartment syndrome: pain, pallor, pulselessness, paresthesia, paralysis.
- Ensure proper skin care and fit for children wearing a Milwaukee brace for scoliosis.
- Promote a diet high in calcium and vitamin D for bone healing.
- Use a multidisciplinary approach for children with cerebral palsy to maximize function.
Patient teaching
- Instruct parents not to use lotions or powders under a Pavlik harness.
- Teach children with scoliosis braces to wear a thin cotton t-shirt under the brace.
- Explain the importance of physical therapy exercises for children with cerebral palsy.
- Advise parents not to stick objects down a cast to scratch an itch.
