Pediatric Neurological Disorders
Overview
Neurological disorders in children encompass a wide range of conditions, from congenital neural tube defects to acquired infections and seizure disorders. Because the pediatric brain is still developing, injuries can have long-lasting effects on cognition and motor function. Nursing care centers on neurological monitoring, maintaining safety during seizures, and managing intracranial pressure.
Key nursing concepts
- The infant's open fontanels allow for some compensation for increased ICP.
- Seizure management focuses on safety and airway protection.
- Hydrocephalus involves an imbalance in CSF production and absorption.
- Meningitis requires immediate droplet precautions and antibiotic therapy.
- Spina bifida requires meticulous care of the protruding sac to prevent infection.
- Neurological assessments include the Glasgow Coma Scale modified for age.
Important nursing considerations
- Maintain seizure precautions (padded side rails, suction at bedside) for at-risk children.
- Observe and document the start time, duration, and body movements during a seizure.
- Assess for signs of increased intracranial pressure (ICP), such as a high-pitched cry.
- Monitor head circumference daily in infants with hydrocephalus.
- Keep the child in a side-lying position during the postictal phase to prevent aspiration.
- Check for CSF leakage (halo sign) in children with head trauma or VP shunts.
Patient teaching
- Teach parents never to give aspirin to children with fever or viral symptoms.
- Instruct caregivers on how to recognize signs of VP shunt malfunction or infection.
- Explain the need for helmets in children with frequent drop-attack seizures.
- Advise parents that most febrile seizures are benign but still require evaluation.
